Manuel Ugarte scored the winning goal in the fifth round of the shootout, sending Uruguay through and condemning Brazil to a runners-up finish in Group A.
Brazil had the better chances in the 90 minutes, but they were unable to find a way past a resolute Colombia defence.
Neymar came closest to breaking the deadlock in the second half, but his shot from just outside the area was well saved by David Ospina.
The draw means that Brazil will now face Uruguay in the quarter-finals, while Colombia have topped Group D and will play Peru in the last eight.
The quarter-finals will be played on Thursday and Friday, with the semi-finals taking place on Sunday and Monday.
The final will be held on Saturday, July 16, at the Maracana Stadium in Rio de Janeiro.
